Job Summary

We are looking for a motivated and patient-centered Physical Therapist who can provide exceptional care to patients in the comfort of their own homes. As a member of our team, you will have the opportunity to work with a diverse range of patients and develop your skills in a dynamic and supportive environment.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, assisting the physician in developing a comprehensive plan of care.
  • Develop and implement individualized treatment plans, using evidence-based practices to promote optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Monitor patients' progress, making adjustments to treatment plans as needed to achieve desired outcomes.
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • A valid driver's license and reliable transportation.
