Job Title: Transportation Coordinator - Hourly

This role plays a vital part in ensuring the safety and well-being of our residents during medical visits. If you have a passion for caregiving and strong communication skills, we encourage you to apply for this position.

Responsibilities:

  • Ensure the safe transportation of residents to and from medical appointments, including hospitals and doctor's offices.
  • Accompany residents on all medical visits and remain with them until the visit is complete.
  • Obtain and review resident documents, prescriptions, and appointment cards as necessary.
  • Communicate effectively with office personnel, doctors, and other healthcare professionals to ensure seamless care coordination.
  • Perform other duties as required and assigned by management.

Requirements:

  • Valid state driver's license and clean driving record.
  • Physically able to assist ambulatory and non-ambulatory residents transfer to and from wheelchairs to vehicles.
  • Certified Nurse Assistant (C.N.A.) or equivalent experience.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
