Physician Assistant for Post-Acute Rehabilitation

We are Resilient Healthcare, a leading healthcare organization dedicated to providing exceptional care and support to our patients. We are currently seeking an experienced Physical Therapist to join our team as a part-time (PRN) member.
Estimated Salary Range: $60,000 - $80,000 per annum.
Job Description:As a Physical Therapist at Resilient Healthcare, you will play a vital role in the rehabilitation of patients who require post-acute care. Your primary responsibility will be to design and implement individualized treatment plans that cater to the unique needs and goals of each patient.
Key Responsibilities:
- Patient Assessment and Goal Setting: Conduct thorough assessments to identify patients' strengths, weaknesses, and functional limitations, and set realistic goals that align with their needs and abilities.
- Treatment Planning and Implementation: Develop and execute evidence-based treatment plans that address patients' physical, emotional, and social needs, and provide them with the skills and confidence to achieve optimal function and independence.
- Clinical Supervision and Collaboration: Work closely with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure seamless care coordination and effective communication.
- Documentation and Record Keeping: Maintain accurate and comprehensive records of patient progress, treatment plans, and outcomes, using standardized documentation tools and software.
Requirements and Qualifications:
- Education and Certification: Master's or Doctoral degree in Physical Therapy from an accredited institution; current licensure to practice as a Physical Therapist in the relevant state.
- Experience and Skills: Proven experience working in post-acute settings; excellent clinical skills, with expertise in assessment, treatment planning, and implementation; strong communication and interpersonal skills, with ability to work effectively with patients, families, and multidisciplinary teams.
