Job Description

About the Job: We are seeking a highly motivated and compassionate Direct Support Professional to join our team at Friendship Community Care. As a key member of our interdisciplinary team, you will be responsible for delivering exceptional quality of service and providing appropriate activities in a safe and healthy environment within the home and community setting.

Responsibilities:

  • Assist clients in basic living skills and daily personal tasks, including showers, dressing, housekeeping, grocery shopping, meal preparation, and financial budgeting.
  • Provide transportation to and from activities and appointments as needed, requiring a valid Arkansas driver's license.
  • Maintain good communication via facility logbook, staff meetings, and other means between staff regarding client needs and progress toward their plan of care by the end of shift.
  • Accurately complete all documentation required by FCC, including behavioral reports, incident reports, compliance forms, personal care work sheets, requisitions, maintenance forms, progress reports, monthly attendance logs, and other necessary documents.
  • Fully comply with FTA vehicle requirements and maintain accurate mileage and billing records in accordance with Medicaid guidelines.
  • Report incidents and emergencies to supervisor or delegated authority immediately, as this is a safety-sensitive position.
  • Assist in formulating, managing, revising, and following through on client plans of care to ensure treatment is carried out and good health, safety, and security are prioritized.
  • Follow proper medication management guidelines and monitor/document these procedures.
  • Organize and carry out recreational activities for clients.
  • Strive for good role modeling for clients through adherence to FCC's dress code and appropriate actions.
  • Treat clients and their families with dignity and respect at all times, reflecting positively on FCC and its mission.
  • Contribute to the maintenance of FCC facilities and grounds, keeping them clean, orderly, and functional.
  • Support the maintenance of FCC vehicles, ensuring cleanliness and regular preventive maintenance.
  • Participate in necessary training and continuing education.
  • Provide proof of liability insurance and maintain current coverage.
  • Possibly undergo periodic driving tests and demonstrate proficiency in operating a motor vehicle, including passenger vans, and may drive personal vehicles for transport when necessary, maintaining adequate liability coverage.
  • Adhere to confidentiality policies regarding Friendship Community Care, Senior Center Programs, and Clients, aligning with Friendship Community Care Non-Disclosure Policy and HIPAA Policies.
