Responsibilities- Provide exceptional patient care and support to individuals with behavioral health needs.
- Collaborate with the Case Management team and other departments to ensure efficient and effective care management.
- Work closely with physicians and other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments and develop comprehensive care plans to meet the unique needs of each patient.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Bachelor's degree in Nursing or related field.
- Two years of acute Case Management experience within the past 5 years.
- Preferred: Case management experience in a field related to behavioral health.
