Optometrist Opportunity

We are seeking an experienced Optometrist to join our team as an Independent Eye Care Professional. As an Independent Optometrist, you will have the autonomy to establish your own practice within our Sam's Club locations.

• Provide high-quality eye care services to patients, including comprehensive eye exams and vision correction.
• Establish your own schedule and fee structure, with the option to lease equipment and administrative support services.
• Work closely with our team to ensure seamless integration with our services and benefits.

• Doctor of Optometry (OD) degree from an accredited optometry school.
• Valid state optometry license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.
