About This Role

We are seeking a compassionate Certified Home Health Aide to join our team at VNS Health Personal Care. As a key member of our interdisciplinary team, you will provide essential support and care to clients in their homes.

Key Responsibilities

  • Assist clients with daily living activities such as ambulation, transfer, ostomy care, and medication management.
  • Perform light housekeeping tasks, including tidying, shopping, and laundry.
  • Monitor client condition, report changes, and maintain accurate records.
  • Adhere to infection control practices and assist with nutritional and fluid intake needs.
  • Ensure client rights and confidentiality are maintained.

Qualifications

  • Satisfactory completion of New York State Health Department approved HHA Training Program or Equivalency.
  • At least 6th grade reading comprehension level.
  • Ability to write and perform math functions necessary for job duties.
  • Age 18 years or older.

Physical Requirements

  • Full range of body motion, including handling and lifting adult clients.
  • Standing and walking for extended periods, with occasional bending and climbing stairs.
  • Lifting items weighing more than 50 pounds.
