Job SummaryWe are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team of home care professionals. As an LPN with Mass General Brigham Healthcare at Home, you will have the opportunity to work with a diverse patient population, providing care that is tailored to their individual needs.
Responsibilities- Provide high-quality, patient-centered care in the home setting
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care outcomes
- Participate in ongoing education and training to stay current with best practices and industry developments
- LPN professional license in Massachusetts
- Minimum 1 year previous nursing experience in acute care or sub-acute care setting
- Home care experience preferred
- Effective written, verbal, and electronic communication skills
- Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred
