We are seeking a dedicated Medical Social Worker to join our team at Avera@Home Yankton. As a key member of our multidisciplinary team, you will play a vital role in providing medical social casework to patients of all ages.

Job Summary

The successful candidate will provide comprehensive medical social services, including assessment, counseling, and coordination of community resources. You will work closely with healthcare teams to develop and implement individualized plans of care that meet the unique needs of each patient.

Key Responsibilities

  • Provide medical social casework to patients and their families, addressing psychosocial and continuum of care needs.
  • Assess patients' needs and develop individualized plans of care, in collaboration with healthcare teams.
  • Coordinate community resources and services to support patients' well-being.
  • Maintain accurate records and communicate effectively with healthcare teams and other stakeholders.

Requirements

To be considered for this rewarding opportunity, you must possess a Master's degree in Social Work (MSW) and hold current licensure as a Social Worker in the state of Nebraska or Iowa, South Dakota, Minnesota, or North Dakota.

Work Schedule

This full-time position offers an 80-hour biweekly schedule, Monday through Friday, from 8:00 am to 4:30 pm.
