Urrly is seeking a Nurse Practitioner to lead a care model that's revolutionizing the way nursing home residents receive care. As a key member of our team, you'll focus on keeping patients healthier and happier in place, reducing unnecessary hospital trips, and delivering seamless, personalized care.

Your Mission
  • Develop and implement a care model that prioritizes patient-centered care and quality outcomes.
  • Manage your own patient panel, handle advanced care planning, and coordinate with a passionate care team to deliver outstanding results.
  • Streamline clinical processes, such as medication reviews and prior authorizations, while ensuring quality care metrics are met.
What You Bring
  • Unrestricted Advanced Practice Nurse license and national board certification.
  • Eligibility for Medicare participation.
  • 6+ months of experience as an Advanced Practice Provider, preferably in geriatric, hospice, or palliative care.
  • Strong clinical skills and experience with chronically ill patients, ideally in a nursing home setting.
  • Tech-savvy with excellent communication skills.
