Join Our Team as a Physical Therapist in Home Health

We are seeking a dedicated and compassionate Physical Therapist to join our team at Geisinger Home Health in Lewistown, Pennsylvania. As a Physical Therapist, you will play a vital role in helping patients achieve their maximum potential and improving their overall quality of life.

About the Role

As a Physical Therapist in Home Health, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, you will work with the patient and their family to develop a personalized treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

Responsibilities
  • Evaluate patient's functional status and physical therapy needs, and assist the physician in developing the therapy plan of care.
  • Observe and report patient's response to treatment and any changes in their condition to the supervising nurse and/or the physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and their family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in the state of Pennsylvania.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
