Licensed Practical Nurse

Avail Senior Living is seeking a skilled Licensed Practical Nurse to join our team. As a key member of our care team, you will be responsible for providing high-quality resident care and supervision to our residents.
Key Responsibilities- Provide direct resident services and medication management as needed.
- Assist in the implementation of and/or provide additional information for the resident service plan and update them on an ongoing basis.
- Assist care providers with the implementation of service plans.
- Oversee the provision of assistance with ADLs to residents whose service plan indicates the need.
- Communicate pertinent resident information to co-workers when leaving the community and/or at shift changes.
- Assume responsibility for quality resident care and seek guidance from supervisory staff when necessary.
- Communicate with physicians/other health providers regarding the resident's health status when appropriate.
- Determine when a resident requires room service for meals due to illness.
- Maintain accurate and complete resident file documentation.
- Schedule tests ordered by the physician, assist residents in scheduling medical appointments and transportation.
- Visit residents who are ill and provide/arrange necessary follow up.
- Respond immediately to emergency situations or resident calls.
- Welcome and orient new residents to the building and services available in the community.
- Provide educational support and guidance to residents and their families.
- Assist with monthly Wellness checks.
- Perform other duties as directed.
- Assist with ADL assessments.
- Take calls as directed by wellness director.
- High school graduate or equivalent. Must have a valid, unrestricted nursing license in the state.
- Must demonstrate competency in the following areas:
- Staff supervision
- Resident care skills
- Medication management principles and practices
- Written and verbal communication skills
- Current CPR certification
- Maintain CEU requirements
- Must have the strength and endurance required to sit, bend, lift, and move intermittently during working hours.
- Works beyond normal hours, weekends, evenings, holidays, severe weather situations when necessary.
- Is subject to potential exposure to infectious waste, disease, conditions, etc., including TB, HIV, and Hepatitis B.
