Job Description

We are seeking a skilled Hospice Nurse Practitioner to join our team. As a face-to-face encounter specialist, you will be responsible for gathering critical clinical information for hospice diagnosis and prognosis. You will conduct physical assessments, provide clinical findings, and communicate with physicians to ensure patient eligibility for hospice care.

Key Responsibilities
  • Conduct face-to-face encounters to gather clinical information for hospice diagnosis and prognosis.
  • Review medical treatment plans, diagnostic tests, lab results, and medication lists to inform clinical decisions.
  • Communicate clinical findings to the certifying physician to ensure patient eligibility for hospice care.

Requirements

To be successful in this role, you must have a Master's or higher degree in Nursing, completion of a graduate-level nurse practitioner education program accredited by a national accrediting body, current state license as a registered nurse, current state license or certificate as an advanced practice nurse, and current national certification as a Nurse Practitioner (ANCC or AANP). You must also have a collaborative agreement in place.
