Licensed Practical Nurse - Inpatient Medical Unit

This role is for a full-time LPN working either day shifts (7am-7pm) or night shifts (7pm-7am), with a rotation for weekends and vacations.

The Inpatient Medical Unit is a specialized 25-bed facility catering to a diverse range of general medical patients, with a particular emphasis on respiratory conditions, renal complications, altered mental states, and gastrointestinal issues. Here, you will encounter a wide spectrum of patients with significant medical needs. The collaborative spirit among the staff is exceptional, and you will quickly appreciate this dynamic after just a few shifts.


As an LPN in this environment, you will deliver care to a designated group of patients, applying the skills acquired through your training and licensure.

With the guidance of an RN and support from the care team, your responsibilities will include:

- Delivering safe and dependable patient-centered care under the supervision of an RN.
- Collaborating with a multidisciplinary team.
- Conducting ongoing assessments and monitoring vital signs.
- Administering medications as prescribed.
- Providing evidence-based patient education from the point of admission.
- Documenting daily progress towards care objectives.
- Performing bedside procedures (e.g., foley catheter insertion, NG tube placement, dressing changes, drain management, etc.).
- Offering discharge education and reviewing instructions with patients and/or caregivers.
- Maintaining accurate records in the electronic medical system.

Key Responsibilities:
Work in conjunction with the healthcare team to ensure optimal patient care. Consult with the supervising RN and other team members as necessary.

Seek guidance and supervision when implementing new or unfamiliar nursing practices.


Contribute to the admission assessment process by accurately collecting, reporting, and documenting both objective and subjective data within your scope of practice.

(Initial assessments will be conducted by the RN.)


Assist in identifying and supporting a multidisciplinary approach to determine suitable patient care interventions that promote comfort and safety.

Support the healthcare team in achieving patient care outcomes and objectives.

Administer medications and perform tasks within your professional scope.

Work collaboratively with patients and families to anticipate necessary steps for safe transitions in care.

Promptly inform the supervising RN of any changes in patient conditions.

Engage with the RN and other healthcare team members to communicate progress towards established care goals.

Assist in updating care plans to reflect changes in patient needs, under the direction of the supervising RN.

Ensure real-time documentation in patient charts to verify that care goals and outcomes are being met.

Conduct hand-offs with meticulous attention to detail, prioritizing safety.

Communicate with providers during routine rounds and as needed.


Incorporate patient and family education throughout the hospitalization, covering the care plan, pre- and post-operative procedures, diagnostic tests, and discharge instructions.

Integrate preventive health measures into the educational framework.


Assess and select teaching strategies that are appropriate for the age, culture, readiness to learn, and comprehension levels of patients and their families.

Consult relevant resources within the organization to provide specialized education as necessary.

Qualifications:
- Completion of an accredited LPN program.
- Previous experience as a CNA or PCT is preferred.
- Current LPN licensure in the State of Iowa or a compact state.
- Proficient in reading, writing, speaking, and understanding English.


- Demonstrated competency in the manual skills and techniques required for basic nursing procedures and the operation of specialized equipment.

Ability to successfully complete training on the Electronic Medical Record (EMR) system.

Knowledge of the laws governing licensure and the ability to function within the legal boundaries of the profession.

Adherence to established departmental routines, policies, and procedures.

Strong verbal and written communication skills with patients, families, healthcare team members, and other stakeholders.

Real-time EMR documentation in accordance with hospital and unit policies.
