About the Role

As a Nurse Coordinator for Home and Hospice Care at SSM Health, you will have the opportunity to make a meaningful impact in the lives of patients and their families. This role requires collaboration with multidisciplinary teams to ensure quality care delivery, effective communication with patients and families, and adherence to regulatory standards.

This position involves providing skilled nursing services such as wound care, IV therapy, and conducting medical assessments in patients' homes or residential facilities. You will work closely with patients, their families, and healthcare teams to develop personalized care plans that cater to their unique needs.

Key Responsibilities
  • Assess patient needs and develop comprehensive care plans under the direction of attending physicians
  • Collaborate with multidisciplinary teams to ensure seamless care delivery and effective communication
  • Provide skilled nursing services, including wound care, IV therapy, and medical assessments in patients' homes or residential facilities
  • Document visit notes, treatments, and care in a timely and accurate manner
  • Respond to patient calls, address concerns, and provide follow-up support as needed
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure
  • Basic Life Support (BLS) certification from the American Heart Association
  • Registered Professional Nurse (RN) licensure in the state of practice
  • Valid driver's license and a clean driving record
