ResCare Community Living is seeking a skilled Nursing Supervisor to join our team. As a Nursing Supervisor, you will be responsible for overseeing the health and well-being of individuals served in our community living programs.
Key Responsibilities:- Develop and Implement Care Plans: Collaborate with programmatic and behavioral health staff to develop and implement individualized care plans that meet the unique needs of each person served.
- Assess and Identify Health Needs: Conduct regular assessments to identify health needs and develop strategies to address them.
- Supervise Healthcare Team: Supervise and evaluate the performance of LPNs, CNAs, and DSPs related to health services activities.
- Ensure Compliance: Ensure compliance with state, local, and payer requirements through review of documentation and attendance at IDT and care coordination activities.
- Participate in Performance Improvement: Participate in performance improvement activities and maintain ongoing clinical knowledge through internal and external training programs.
- Address Acute and Ongoing Health Issues: Address acute and ongoing health issues and ensure that medical appointments are scheduled and attended as needed.
- Document Actions: Document all actions and decisions in Nurse's Notes or Service Delivery Log.
- Communicate with QIDP/PM: Keep QIDP/PM informed of health needs and the need for IDT meetings.
- Reconcile MARs: Reconcile and place MARs at each group home prior to the last day of the month.
- Complete Quarterly/Monthly Nursing Review: Complete a quarterly/monthly nursing review, including an assessment for each individual.
- Ensure Clinical/Medical Records are Up-to-Date: Ensure that clinical/medical records are up-to-date, including medication, clinical histories, seizure records, weights, immunizations, and medical appointments.
Requirements:
- Nursing Experience: At least one year of nursing experience, including 6 months in a supervisory role.
- Clinical Experience: Clinical experience in providing services and supports to individuals with intellectual and developmental disabilities or related disorders is preferred.
- Degree/Certification: Degree/certification from an accredited school of nursing.
