Duties and Responsibilities
  • Monitor clients' conditions and provide individualized care
  • Follow up with and execute doctors' orders
  • Perform client assessments and develop personalized care plans
  • Coordinate case management and communicate effectively with healthcare teams
  • Accurately document observations, interventions, and evaluations
  • Travel to clients in your local area on a schedule that suits your needs
Qualifications and Requirements

To be eligible for this role, you must:

  • Hold a current nursing license in good standing in New Jersey
  • Have graduated from an accredited and approved nursing program, as indicated by school transcript or diploma

$95,000 - $110,000 per year (estimated salary range based on location and experience). This estimate may vary depending on individual circumstances.
