Job Summary

UC San Diego Health is seeking a skilled RN Care Manager to join our team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring the best possible outcomes.

Key Responsibilities
  • Facilitate patient hospitalization from pre-admission through discharge
  • Coordinate with physicians, nurses, social workers, and other healthcare providers to expedite medically appropriate cost-effective care
  • Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning
  • Communicate with administration and the healthcare team as needed to facilitate patient care
Requirements
  • Graduate from an accredited school of Registered Nursing
  • Registered Nurse (RN) license issued by the State of California
  • Current UCSD BART certification or BLS at time of hire with commitment to get BART within six (6) months of hire date
  • Prior RN Case Management experience in an acute care hospital
  • Demonstrated utilization review/discharge planning experience
  • Knowledge of payer industry, resource management, reimbursement, and evidence-based clinical practice
  • Proficiency in Information Systems, databases, and computer programs including MS Office: Word, Excel, Outlook, PowerPoint, etc.
  • Proven fast and accurate typing and data entry skills
  • Strongly skilled in conflict management and resolution
  • Must possess strong interpersonal skills, leadership, negotiation skills, and knowledge of hospital operations
Preferred Qualifications
  • BSN/MSN
  • ACM and/or CCM certificate
  • Three to five (3 - 5) years clinical RN experience or RN care management experience in an acute care setting
  • Previous supervisory/management experience
  • Knowledge of local payers and coverage, reimbursement, and evidence-based clinical nursing practice
Working Conditions
  • Must be able to work various hours and locations based on business needs
  • Employment is subject to a criminal background check and pre-employment physical

Pay Transparency Act

Annual Full Pay Range: $144,711 - $190,885 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $69.31 - $91.42
