Job Title: Hospice RN Case Manager

At Interim HealthCare Staffing, we are seeking a compassionate and skilled Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Key Responsibilities:

  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct comprehensive assessments of patients and their families, identifying their unique needs and developing personalized care plans.
  • Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care delivery and achieve the goals of hospice care.
  • Coordinate services with other healthcare providers, ensuring continuity of care and optimal patient outcomes.
  • Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and their families.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with preference given to those with precepting or training experience.
  • CPR certification and ability to provide proof of valid driver's license and auto liability insurance.
