About the Role

We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Park Place Care Center. As a key member of our staff, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Schedule residents for assessments and ensure timely completion of the comprehensive assessment (MDS)
  • Monitor and maintain accurate and up-to-date documentation of resident care plans
  • Coordinate with nursing and other departments to ensure seamless communication and care delivery
  • Attend regular Medicaid meetings to discuss resident care and updates
  • Perform transmissions of the MDS on a timely basis
Requirements
  • Licensed Nurse in good standing
  • Geriatric nursing background and restorative care philosophy
  • Excellent professional skills, including assessment, hands-on care, and communication
  • Working knowledge of nursing home regulations
  • Ability to work independently and as part of a team
