We are seeking a skilled MDS Coordinator to join our team at SSM Health. As a key member of our healthcare team, you will play a vital role in ensuring the accuracy and quality of resident care plans.
Responsibilities- Collaborate with interdisciplinary teams to develop and implement resident care plans.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions.
- Monitor and analyze Minimum Data Set (MDS) data to identify trends and areas for improvement.
- Develop and implement training programs for nursing staff to enhance their skills and knowledge in resident care.
- Ensure compliance with regulatory requirements and industry standards.
- Graduate of an accredited nursing program.
- Two years of experience in geriatric care, with a focus on Medicare A coordination.
- Basic Life Support (BLS) certification.
- Registered Nurse (RN) licensure in the state of employment.
