Job Description
">As a Home Health Clinical Operations Leader - Physical Therapist (PT), you will be responsible for ensuring ongoing interdisciplinary assessment of the patient, supervising paraprofessional and licensed professional team members, and using clinical decision making to efficiently and effectively manage an assigned patient population.

Key Responsibilities:
">- Ensure safe, effective, and efficient clinical practice through support and oversight of home care staff in accordance with federal and state laws, rules, and regulations.
">- Develop and implement education plans for patients and their families to promote progress toward mutually established goals.

Required Skills and Qualifications
">To be successful in this role, you must have a graduate degree from an accredited Physical Therapist Program, active PT license in Colorado, and at least three years of experience in physical therapy in home healthcare. Additionally, you should have two years of management experience and two years of OASIS experience, preferably.
