Responsibilities:- Implement and manage individualized care plans for patients, ensuring their physical, emotional, and social needs are met.
- Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' conditions, identifying areas for improvement and developing strategies to address them.
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care.
- Document patient progress, including medical history, treatment plans, and outcomes.
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.
- Current, unencumbered LVN license in the state of practice.
- Minimum 6 months of experience as an LVN in a clinical care setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
- Basic life support (BLS) certification.
