ConcertoCare is seeking a skilled Licensed Vocational Nurse to join our team as a full-time Home Care Specialist. As a key member of our care team, you will deliver clinical care to patients in their homes and provide virtual clinical care support.
Key Responsibilities- Provide direct nursing care to patients through routine and urgent in-home and virtual visits under the direction of an authorized healthcare practitioner.
- Facilitate virtual visits in the patient's home to support care delivery under the direction of a remote clinical practitioner.
- Support clinical visits as directed, including chart preparation, clinical intake, after-visit care coordination, and clinical documentation and order entry support.
- Engage patients in their healthcare journey, establish meaningful connections, and provide care tailored to the patient's needs.
- Perform various clinical skills, including blood draws, laboratory specimen collection, medication and vaccine administration, and basic hearing and vision tests.
- Current licensure as a Licensed Vocational Nurse (LVN) in the state of practice.
- Experience in home-based care delivery, urgent care, or emergency room settings.
- Ability to travel 75%+ of the time within an assigned area.
- Strong computer and software skills, with ability to learn new systems quickly.
- Ability to work independently and as part of a multidisciplinary team.
