Job DescriptionWe're seeking a Nurse Practitioner or Physician Assistant to join our home-based primary care team in Southeast Michigan. This role will support our vulnerable Medicare and Medicare advantage patients, requiring travel to various locations.
Key Responsibilities- Provide high-quality, patient-centered care to adult patients with acute, chronic, and potential health problems.
- Conduct comprehensive health histories and physical assessments, and develop treatment plans.
- Work autonomously, providing care without physician supervision.
- Collaborate with our team to ensure seamless care coordination and population health management.
- Graduate of an accredited advanced practice program and certified as a Nurse Practitioner or Physician Assistant.
- At least two years of primary care or home care experience as an Advanced Practice Provider.
- Current, unrestricted State license and valid drivers license.
- Active DEA registration and Medicare/Medicaid enrollment.
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
- Reduced administrative burden and support.
- Easy-to-use EMR with full telehealth capability.
