Job Summary

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Willowbrook Hospice. As a key member of our Interdisciplinary Team, you will provide essential social services to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' psychosocial and spiritual needs, developing personalized plans of care in collaboration with the Interdisciplinary Team.
  • Insurance and Financial Support: Evaluate patients' insurance coverage and financial status, referring them to relevant agencies for assistance as needed.
  • Social Service Activities: Provide counseling, support, and education to patients and their families, promoting their well-being and quality of life.
  • Team Collaboration: Participate in Interdisciplinary Team meetings, ensuring seamless communication and coordination of care.
  • Bereavement Support: Offer bereavement follow-up services to patients' families, providing emotional support and guidance during a difficult time.
  • Record Keeping: Maintain accurate, complete, and timely records of all activities and services provided to patients and their families.
  • Training and Orientation: Assist in the orientation and training of volunteers, staff, physicians, and community members, promoting a culture of excellence and compassion.
Requirements
  • Education: Master's degree from a school of social work accredited by the Council on Social Work Education.
  • Experience: At least two years of experience working with geriatric populations in a healthcare setting, with a preference for hospice experience.
  • Skills: Strong communication and interpersonal skills, with the ability to relate positively to diverse personalities and family dynamics.
Working Conditions
  • Physical Demands: Ability to access patients' homes and all rooms therein, as well as drive a car.
  • Communication: Effective verbal and written communication skills, with the ability to communicate with patients, families, and healthcare professionals.
