Job SummaryThis is an in-person position responsible for covering weekend needs and two holidays per calendar year. The provider will be responsible for rounding and managing the clinical care of the population at the assigned Skilled Nursing Facility (SNF). The provider will provide high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
Key Responsibilities- Rounding and managing the clinical care of the population at the assigned SNF
- Prescribing medications and treatments for residents residing at SNFs
- Providing high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs
- Timely documentation of all patient care/encounters
- Availability for phone/secure messaging consultations with facility nursing staff
- Valid PA/NP license in the state in which they will practice
- Valid NP/PA degree from an accredited institution
- Active DEA license (Theoria will assist with application for new graduates)
- General Post-Acute/SNF knowledge or experience preferred
- Familiarity with EMR systems and charting practices
- wRVU productivity
- Malpractice insurance covered by employer
- Opportunities for training (i.e. wound care)
- All practice expenses covered
