Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at the Hudson Valley DDSO - Human Resources. As a Licensed Practical Nurse, you will provide nursing services to individuals with developmental disabilities residing in our group homes.

Responsibilities:
  • Monitor the health status of individuals and provide appropriate interventions.
  • Administer medications and treatments as prescribed.
  • Order medications and medical supplies as needed.
  • Review and transcribe physician's orders.
  • Complete semi-annual nursing summaries and attend reviews.
  • File medical reports and maintain accurate medical records.
  • Supervise and provide direct care services, including ADLs.
  • Provide in-service training for Direct Support Assistant Trainees.
  • Perform personal care of consumers residing in the home.
  • Monitor the conditions of individuals in their assigned worksite and report any changes or symptoms to a physician or registered nurse.
Requirements:
  • A license and current registration to practice as a practical nurse in New York State, or a limited permit to practice as a practical nurse in New York State, or an application on file for a limited permit to practice as a practical nurse in New York State.
  • A valid Driver's license and dependable transportation.
  • Ability to work in a fast-paced environment and provide excellent customer service.
  • Strong communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
