Key Responsibilities:
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and facility policies.
- Administer medications and treatments as prescribed by physicians, ensuring patient safety and well-being.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct regular assessments and evaluations to identify potential health risks and develop strategies to mitigate them.
- Provide education and support to patients and their families, promoting health literacy and self-advocacy.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Supervise and mentor subordinate staff, promoting a culture of excellence and professionalism.
Requirements:
- Current, unrestricted LPN license in the state of practice.
- Previous skilled nursing experience, preferably in a long-term care setting.
