Job Description

We are seeking a highly skilled and compassionate Primary Care Physician to join our team. As a member of the PACT, you will be responsible for managing a panel of assigned veterans, providing comprehensive primary care services, and collaborating with other healthcare professionals to ensure seamless care coordination.

Responsibilities:

  • Manage a panel of assigned veterans, providing comprehensive primary care services.">
  • Deliver preventative care, including health screenings, immunizations, and counseling.">
  • Accurately diagnose and treat acute and chronic illnesses, including managing complex health conditions.">
  • Collaborate effectively with other healthcare professionals, including specialists, nurses, and pharmacists, to ensure seamless care coordination.">
  • Educate patients and their families about health conditions, treatment plans, and self-management strategies.">
  • Participate in quality improvement initiatives and contribute to the overall excellence of the PACT.">

Required Skills and Qualifications

To be successful in this role, you must possess the following skills and qualifications:

  • MD or DO degree: Graduation from an accredited medical school is required.">
  • Board Certification or Eligibility: Certification by the American Board of Family Medicine or the American Board of Internal Medicine is required or eligibility for certification must be demonstrated.">
  • State Medical License: Must possess a valid, unencumbered medical license in the state of Texas.">
  • Clinical Expertise: Strong clinical skills and a demonstrated ability to deliver high-quality primary care services.">
  • Communication & Interpersonal Skills: Exceptional communication and interpersonal skills, enabling effective patient interaction and building strong patient relationships.">
  • Teamwork Proficiency: Must be able to work both independently and collaboratively as part of a multidisciplinary healthcare team.">
