Clinical Care Specialist

About Us
We are present in the full breadth of the continuum, from patient's homes and virtual spaces to skilled nursing facilities, assisted living, and acute care hospitals. Our team includes Intensivists, Oncologists, Geriatricians, Family Medicine, Emergency Medicine, Internal Medicine, Nephrologists, and Hospice and Palliative Care Physicians and Nurse Practitioners.
Your Role
You will display expertise in providing symptom management and ongoing goals of care discussions with our patients. You will possess excellent communication skills, working with seriously ill and/or terminal patients and their families. You will work closely with the medical director, team physicians, and team leaders to provide continuity of care consistent with the hospice philosophy and the patient's wishes.
Responsibilities
- Provide symptom management and ongoing goals of care discussions with patients.
- Work effectively with all members of the hospice team.
- Call responsibility is included in the rounding responsibility when providing visits in an inpatient setting.
Qualifications
Maintains an active License and DEA in the state where care is being provided.
Estimated Salary
$120,000 - $180,000 per year, depending on experience.
