Case-Manager for Youth and Family Support

We are seeking a dedicated and passionate professional to join our team at Apex Solutions in Cologne Mülheim. As a Case-Manager for Youth and Family Support, you will play a vital role in promoting the well-being of children, adolescents, and families with diverse needs.

Key Responsibilities:
  • Develop and implement individualized support plans for children and adolescents with special needs.
  • Collaborate with schools, social services, and community organizations to ensure comprehensive support for families.
  • Provide social group work and individual support to children and adolescents, focusing on youth and family support in the community.
  • Contribute to the development of inclusive education programs and materials.
  • Work closely with the team to ensure seamless support for children and adolescents with diverse needs.

Requirements:

  • Completed training as an Educator or Childhood Education, or a Bachelor of Arts in Social or Special Education or Social Work.
  • Practical experience in social work with a focus on youth and family support in the community.
  • Knowledge of social group work and individual support for children and adolescents.
  • Creativity, artistic, or physical skills to bring into your work with children and adolescents.
  • Ability to respond openly and quickly to changes and unexpected events.
