Job Title: Heart Failure Nurse Clinician

UI Heart and Vascular Center is seeking a dedicated and compassionate Heart Failure Nurse Clinician to join our multidisciplinary team. As a key member of our Advanced Heart Failure Program, you will play a vital role in providing comprehensive care and support to patients facing complex cardiac conditions.

Responsibilities:
  • Conduct thorough patient assessments and develop individualized care plans in collaboration with the multidisciplinary team.
  • Manage symptoms of advanced heart failure, including dyspnea, edema, fatigue, and chest pain.
  • Administer medications, including intravenous therapies, and provide patient education on medication management and potential side effects.
  • Collaborate with social workers, case managers, and community resources to address psychosocial needs, financial concerns, and access to support services for patients and their families.
  • Facilitate continuity of care and coordinate patient flow through the UIHC continuum of care on both the inpatient and outpatient setting.
  • Provide ongoing support and counseling to patients and families, addressing emotional and psychological concerns related to living with advanced heart failure.
  • Participate in interdisciplinary rounds, case conferences, and team meetings to discuss patient progress, treatment plans, and care coordination efforts.
  • Document patient assessments, interventions, and outcomes accurately and comprehensively in electronic medical records, ensuring compliance with regulatory standards and organizational policies.
Requirements:
  • Baccalaureate Degree in Nursing is required.
  • Minimum 6 months providing direct nursing care or management/supervision of cardiovascular medical/surgical patients OR Minimum 1 year providing direct nursing care/care coordination within an outpatient setting required.
  • Excellent written, verbal, and interpersonal communication skills.
  • Experience and skills with electronic health record is required.
  • Current certification in BLS required by start date.
  • Current license to practice nursing in the State of Iowa required by start date.
Desired Qualifications:
  • 3 years providing direct nursing care or management/supervision of cardiovascular medical/surgical patient population.
  • Experience managing the care coordination of patients in an outpatient setting desired.
  • Experience in an academic medical setting desired.
  • Previous experience using EPIC desired.
