Key Responsibilities- Assess and Develop Care Plans: Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop personalized care plans to meet their unique requirements.
- Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, hospitals, long-term care facilities, and other settings, as needed.
- Collaborate with Interdisciplinary Teams: Work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive care and support for patients and their families.
- Communicate Effectively: Maintain open and empathetic communication with patients, families, and healthcare teams to ensure seamless care coordination and address any concerns or needs.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes, in accordance with regulatory requirements and agency policies.
- Participate in Quality Improvement: Contribute to quality improvement initiatives and participate in peer review and other quality assessment activities to ensure high-quality patient care and outcomes.
- Current RN Licensure: Possess a current and active RN license in the state of Georgia.
- Experience in Hospice Care: Have a minimum of one year of experience in hospice care or a related field, with a strong understanding of end-of-life care principles and practices.
- Strong Communication and Interpersonal Skills: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to Work Independently: Possess the ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care and support.
- CHPN Certification: Possess a Certified Hospice and Palliative Nurse (CHPN) certification, or be willing to obtain it within a specified timeframe.
- Experience with Electronic Medical Records: Have experience with electronic medical records (EMRs) and other healthcare software systems.
- Strong Analytical and Problem-Solving Skills: Demonstrate strong analytical and problem-solving skills, with the ability to think critically and make sound decisions in complex situations.
