  • Possession of a valid driver's license and willingness to travel as required.
  • Proficient in computer usage, internet navigation, and database management.
  • Availability for on-call duties as necessary.
  • Flexibility to work evenings and weekends on occasion.
Responsibilities

The incumbent will be responsible for:

  • Administering the Shared Parenting program, ensuring adherence to all regulatory requirements, including enrollment, documentation, and problem-solving.
  • Maintaining accurate and up-to-date documentation for individuals receiving Medicaid services, including preparation for audits.
  • Establishing appropriate goals and documentation through the Person Centered Plan (PCP) for all participants in Medicaid-funded programs.
  • Providing education to Direct Support Professionals on effective techniques and methodologies as outlined in the PCP.
  • Reviewing Service Notes, Incident Reports, and Recipient Rights Complaints for completeness and accuracy, offering guidance to ensure proper documentation.
  • Protecting the rights of all individuals by adhering to established procedures and guidelines.
  • Serving as a backup for other coordinators within the Operations team.
  • Participating in agency fundraising initiatives.
  • Attending necessary training sessions and meetings.
  • Carrying an emergency phone as required.
  • Performing additional duties as assigned.
