Key Responsibilities
- Analyze medical information from patient records and accurately code diagnostic and procedural data following national coding guidelines and reimbursement protocols. Collaborate with healthcare providers to clarify any missing or insufficient record information and determine the correct diagnostic and procedure codes.
- Abstract clinical diagnoses and procedure codes, ensuring that all relevant information from the medical record is accurately documented in the electronic medical records system. Identify and rectify any missing information to create comprehensive records, including disease and procedure codes, discharge details, and physician information.
- Conduct quality assurance for medical records, ensuring compliance with coding regulations set forth by various regulatory agencies and professional standards.
- Compile and analyze data from medical records for reporting and research purposes as assigned.
All tasks must be performed in accordance with established policies, procedures, and regulatory requirements, while consistently delivering excellent service to all stakeholders.
Qualifications
A high school diploma or GED, or equivalent specialized training in medical record keeping principles, anatomy, physiology, and medical terminology is required. An Associate's degree in a related healthcare field is preferred.
Candidates must demonstrate knowledge of ICD and CPT coding principles, ideally evidenced by certification from recognized professional organizations. A minimum of six months of coding experience in diverse healthcare settings is necessary. Proficiency in office and coding software is essential.
Preferred Qualifications
Certification as a Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT), Certified Coding Specialist (CCS), or Certified Professional Coder (CPC) is preferred. Experience may be considered in lieu of certification.
