We are seeking a Scheduling Coordinator to enhance our team.

Key Responsibilities:

  • Coordinate patient appointments and maintain accurate scheduling records.
  • Communicate scheduling updates promptly between office personnel and field staff.
  • Keep an updated client roster with essential information.
  • Update logs and scheduling sheets regularly.
  • Prepare weekly schedules for field staff and distribute them via email.
  • Assist in organizing services for patients.
  • Monitor and manage schedule modifications.
  • Compile weekly visit counts and report findings to the Clinical Supervisor.
  • Facilitate communication among field staff, office staff, and community partners.
  • Participate in on-call rotations as necessary.
  • Support shift confirmations and payroll processes for private duty.
  • Optimize geographic scheduling to minimize travel time and enhance care quality.
  • Accurately input new patient referrals into the system and maintain patient calendars.
  • Act as a liaison among staff, patients, families, physicians, referral sources, and payers.
  • Foster ongoing relationships with current, future, and former clients, their families, and external agencies.
  • Engage in company quality improvement initiatives.
  • Adhere to established policies and procedures, ensuring compliance with local, state, and federal regulations.
  • Answer phone calls efficiently and direct them appropriately.
  • Perform additional duties as assigned by supervisors.

Qualifications:

  • Experience in home health care is required.
  • A high school diploma or equivalent is necessary.
  • Strong communication and public relations skills are essential.
  • A commitment to delivering a world-class customer experience is vital.
  • Reliable internet service with minimum broadband is required.
  • Must be qualified as a Med Tech.
  • Ability to work shifts as part of the care team is necessary.
