Job Summary

RNnetwork is seeking a skilled travel nurse RN to fill a Home Health Case Management position in a dynamic healthcare setting.

Key Responsibilities
  • Provide comprehensive case management services to patients in their homes, ensuring tailored treatment plans and optimal care outcomes.
  • Collaborate with healthcare teams to coordinate patient care, addressing complex medical needs and social determinants of health.
  • Develop and implement individualized care plans, incorporating patient preferences and goals.
  • Conduct regular home visits to assess patient progress, provide education, and address concerns.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Document patient information, progress, and outcomes in a timely and accurate manner.
Requirements
  • Current RN licensure and active registration
  • Experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in electronic health records and other relevant software
