Responsibilities- Implement and monitor patient care plans to ensure optimal outcomes.
- Monitor, record, and communicate patient condition to ensure seamless care coordination.
- Serve as a primary coordinator of all disciplines to provide well-coordinated patient care.
- Note and carry out physician and nursing orders to ensure timely and effective care.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.
- Licensure/Certification/Registration: BLS Provider, Registered Nurse credentialed from the Florida Board of Nursing, Diploma from an accredited school/college of nursing.
- Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
