Key Responsibilities- Respond to patient requests or medical advice questions from the patient portal.
- Respond to patient requests or medical advice questions from patient phone calls (triage) in a timely manner and communicate directly with the patient.
- Review and react to various patient documents, including but not limited to; surgeries, stress tests, CT scans, MRI results, Mammograms, Immunizations, etc.
- Updating of Medications, Problems, Diagnosis, Allergies, and history in the Electronic Medical Record from review of documents.
- Communication with patients' Primary Care Physicians with any significant change in health status.
- Must have a valid NYS Physician Assistant (PA) or Nurse Practitioner (NP) license.
- A minimum of two (2) years' experience working in a family practice or Urgent Care setting and previous experience working with Medent (EMR) is preferred.
- Must be energetic, have an excellent working knowledge of chronic disease states, and possess critical thinking skills.
