Nurse Practitioner

We are seeking a skilled Licensed Practical Nurse to join our primary care team in an ambulatory care setting. As a key member of our team, you will provide high-quality patient care, work collaboratively with healthcare professionals, and contribute to a positive patient experience.
Responsibilities- Provide patient care in accordance with established standards, collaborating with Medical Providers, Registered Nurses, and Medical Assistants to ensure safe, timely, effective, efficient, equitable, and patient-centered care.
- Support the provider by anticipating needs, preparing patients for visits, maintaining efficient patient flow, and assisting with treatments and procedures.
- Ensure patient safety and well-being by identifying and addressing potential risks and concerns.
- Minimum 1 year of experience as a licensed practical nurse from an accredited program.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Patient Safety Advocate: Represent patient concerns and take action to provide safe care.
- Team Care: Participate in interdisciplinary teams to meet patient care and clinic needs.
- Caring Practices: Provide compassionate nursing care to promote comfort and healing.
- Evidence-based Practices: Follow established standards and guidelines to achieve optimal outcomes.
- Patient Education: Provide education to patients and families using identified resources.
- Response to Diversity: Recognize and accommodate cultural, spiritual, and lifestyle differences in provision of care.
- Systems Resources: Effectively use environmental and system resources to provide safe and timely care.
- Clinical Reasoning and Actions: Use clinical reasoning and established guidelines to provide patient care.
- Essential: BLS certification.
- Nonessential: Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Essential: Licensed Practical Nurse (LPN) certification.
