MDS Care Coordinator

We are seeking a skilled MDS Care Coordinator to join our team at Corning Center for Rehabilitation and Healthcare. This is an excellent opportunity to work in a dynamic and patient-centered environment.
About the RoleThis position is responsible for completing accurate assessments, care plans, and supporting activities as assigned. You will also be responsible for maintaining and updating all care plans and assessments, monitoring and auditing clinical records, and informing the DON of any persistent issues related to non-compliant documentation.

- Initiating care plans and supporting activities as assigned
- Maintaining and updating all care plans and assessments
- Monitoring and auditing clinical records
- Informing the DON of any persistent issues related to non-compliant documentation
- Prior experience with MDS 3.0 required
- Valid New York RN License
- Long Term Care Experience Required
- Must be highly organized, professional, and motivated
- Solid computer skills required
- Excellent communication skills
- Friendly and a team worker
As a valued member of our team, you can expect a competitive salary range of $65,000 - $80,000 per year, depending on experience. In addition to a salary, we offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and holidays.
