We are seeking a skilled Nurse Practitioner to join our House Calls team in Idaho and Montana. As a member of our integrated medical team, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow-up care to improve health and well-being.
Key Responsibilities:- Conduct in-home health assessments on enrolled Medicare Advantage members, averaging 45-60 minutes in length.
- Review past medical history, review of systems, medication review, and evidence-based practice screenings.
- Check vitals, conduct a physical exam, and perform point-of-care testing as appropriate.
- Identify diagnoses for care management and active medical management.
- Communicate findings to inform the member's primary care physician of potential gaps in care.
- Identify needs and opportunities for clinical referrals and intervene in urgent and emergent situations.
- Educate members on disease processes, medication, and compliance.
- NP license and national certification (ANCC or AANP) or the ability to obtain national certification and/or NP license in the state of assignment.
- Family, Geriatrics, Adult, Adult-Gerontology Primary, or Adult-Gerontology Acute Care certification.
- Access to reliable transportation to travel to client and/or patient sites within a designated area.
- 1+ years of clinical experience in their highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home Health care or home visit experience.
