Job Summary: We are seeking a skilled LPN Nurse to join our team at Peakview Assisted Living & Memory Care. As a LPN Nurse, you will play a vital role in supporting the health and well-being of our residents.

Key Responsibilities:

  • Provide ongoing clinical monitoring and support to residents
  • Assist community nurses, caregivers, and medication personnel to ensure regulatory compliance and clinical excellence
  • Coordinate resident admission process, including screening, assessment, and care needs
  • Ensure care is delivered as indicated in resident care plans
  • Document and report care provided and resident observations

Requirements:

  • Graduate from an accredited school of practical nursing
  • Current nursing license and state registration (LPN or RN)
  • Three years' experience in assessment and geriatric nursing, preferred
