Job Description

We are seeking a skilled Registered Nurse (RN) to join our team as a Case Manager. In this role, you will perform patient assessments, develop and implement plans of care, and collaborate with our interdisciplinary care team to ensure high-quality patient outcomes.

  • Conduct thorough patient assessments and develop individualized plans of care
  • Collaborate with the care team to achieve patient goals and improve quality of life
  • Provide education and support to patients and their families to promote healthy behaviors and independence
Responsibilities

Your responsibilities will include:

  • Performing on-call duties and providing emergency care as needed
  • Participating in clinical development and continuing education programs to stay current with industry best practices
  • Supervising Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs)
Qualifications

To be considered for this position, you must have:

  • One year of experience as a Registered Nurse (RN) in a clinical setting
  • A valid RN license in the state(s) you are assigned to work
  • Current CPR certification
  • A valid driver's license, reliable transportation, and liability insurance
