Key Responsibilities

  • Conduct regular home visits to monitor patient progress, provide support, and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • RN license in the state of employment.
  • At least two years of experience as a registered nurse, with a preference for home health experience.
  • Current driver's license and reliable transportation.
  • Current CPR and BCLS certification.
