Job Summary

LHC Group is seeking a skilled Occupational Therapist to join our team in providing high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' functional status and occupational therapy needs.
  • Develop and implement individualized plans of care to promote patients' independence and quality of life.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and training to patients and their families on adaptive equipment and techniques to enhance daily living skills.
  • Document patient progress and outcomes in accordance with regulatory requirements and company standards.
Requirements
  • Current Occupational Therapy licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle.
