Job Title: Private Duty Nurse - Aveanna Healthcare

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse to join our team. As a Private Duty Nurse, you will provide one-on-one care to patients in their homes, assisting them with transitioning from hospital to home and providing continuous care to remain at home with their loved ones.

Responsibilities:
  • Implement and update the nursing care plan in collaboration with the patient's physician and healthcare team.
  • Provide skilled nursing care, including administration of prescribed medication, treatments, and therapies.
  • Conduct patient assessments and coordinate care with the healthcare team.
  • Document all activities, assessments, and nursing actions in a timely manner.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
