Occupational Therapist

We are seeking a skilled Occupational Therapist to join our dynamic in-house therapy team at Cloverdale Healthcare Center. As a key member of our team, you will have the opportunity to work with a diverse and interesting adult patient population, providing rehabilitative, skilled, and medically necessary treatment interventions.

• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders, developing appropriate plans of care in accordance with regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practice to support clinical interventions, providing rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders.
• Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and modalities, ensuring effective communication with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summaries of services provided, adhering to regulatory and clinical requirements.
