Key Responsibilities- Resident Care: Provide direct care to residents, including administering medications, performing vital signs, and monitoring their health status.
- Assessment and Evaluation: Conduct regular assessments and evaluations of residents to identify their needs and develop individualized care plans.
- Communication: Maintain effective communication with residents, families, and healthcare professionals to ensure seamless care coordination.
- Documentation: Accurately document resident information, including medical history, medications, and treatment plans.
- Team Collaboration: Work collaboratively with the Resident Care Director and other healthcare professionals to ensure high-quality care and services.
- Education: Graduate of an approved college or school of nursing.
- Licensure: Current state license as a Licensed Practical Nurse (LPN).
- Experience: Minimum of one (1) year experience in a healthcare setting, preferably in a long-term care environment.
- Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in computer skills and electronic medical records.
